What Exactly Is Frozen Shoulder?

Your shoulder is a ball-and-socket joint surrounded by a “capsule” of connective tissue. In a healthy shoulder, this capsule is flexible, allowing the humerus (arm bone) to move freely.

Frozen shoulder occurs when this capsule thickens and becomes tight. Band-like structures called adhesions develop, and there is often a significant decrease in synovial fluid, which acts as the joint’s lubricant. This combination makes movement both difficult and painful.

The Three Stages of Progression

One of the most unique and challenging aspects of frozen shoulder is that it typically progresses through three distinct phases:

  1. The Freezing Stage: You begin to feel a dull ache that intensifies over time. Pain is often worse at night, and you notice a steady decline in your shoulder’s range of motion. This stage can last anywhere from six to nine months.
  2. The Frozen Stage: The pain may actually start to decrease during this phase, but the stiffness becomes profound. Your shoulder feels “locked,” making daily tasks like dressing or grooming nearly impossible. This form of pain can last four to twelve months.
  3. The Thawing Stage: This state is the recovery phase. The range of motion in your shoulder slowly begins to improve, and the joint regains its fluidity.

Why Does It Happen?

While the exact cause isn’t always clear, we find that certain factors increase the risk. Individuals over 40, particularly women, are more susceptible. It is also common following a period of immobilization, such as after a surgery or a fracture, or among those managing systemic conditions like diabetes or thyroid disorders.

How to Manage the Pain

The goal of treatment is to control pain and preserve as much range of motion as possible. While it can be tempting to stop moving the arm altogether, total inactivity can actually worsen the stiffness. We recommend gentle, “pain-free” movements and heat therapy to help relax the surrounding muscles before attempting any stretches.
